Preventing Falls Together Planning Meeting

We worked with the Coordinator of the Preventing Falls Together program to design and facilitate a 2-day planning meeting of the program staff. Because of Horizons’ previous work with Preventing Falls Together, we also provided program history and context to the meeting activities and discussions. Following the meeting, we developed a follow-up report of the discussions and decisions made.
