South Shore District Health Authority: Community Health Planning Support

Horizons worked with the South Shore District Health Authority to support all aspects of the Lunenburg and Queens Community Health Boards (CHBs) Coordinators' community health planning activities. Our work included building the Coordinators' skills in: health promotion and planning; reviewing existing community data and evidence; gathering health plan input through a community survey; collecting, analyzing and using community data; determining the health plan priorities; and developing a template for the resulting plans. The three-year health plans were completed by the CHBs in early December 2006. In early 2007, we acted as a resource for the Coordinators as their plans were presented at the District's Tri-Board Forum
