Yarmouth County Community Health Board
– Community Health Planning Forum

In the spring of 2004, Horizons worked with the Yarmouth County Community Health Board (YCCHB) to design and facilitate a community consultation with a wide range of stakeholders to assess priority issues for the YCCHB to address in its community health plan. We were responsible for the design and delivery of the forum (including presentations on health indicators and the population health approach, and interactive group exercises) and developing a follow-up report summarizing the input and priorities of the stakeholders.
