– Primary Health Care Team Consultations

In the fall and winter of 2004/2005, Horizons conducted a learning needs assessment with Primary Health Care team members across Nova Scotia. The purpose of the focus groups and written survey was to determine Primary Health Care team members’ learning needs in relation to working within a primary health care model of service delivery. Once the needs assessment was complete, we developed and pilot-tested a training module, Working with Communities. The training module was piloted with Primary Health Care teams from across Atlantic Canada.
